Probably an ancestor, but Douman loves to leave these things ambiguous then cut it a third way. (I.E When Kikkuri said "upper floor" ((and it was used before that, I updated the script in the volume version to match it.)) it was very vague in meaning "management" or a physical place above somewhere. Keeping it ambiguous if she was actually from outside the Vivarium or not and it turns out her family are just the contact between the two.)
In Chapter 41, it's mentioned that the earthquake was a long time ago, and not in YokiMum's direct living memory. So there's probably several generations between between Kikkuri and that lady.
But you are right about your implication, because originally - before the entire Kikkuri house purpose reveal - I always believed she was older and I think I mentioned it in a mangadex comment somewhere. She always speaks much more older than the other kids (until she's angry), and she knew how to smoke straight away.
Author got me good with the bait and switch. I fully expected her to have been bullied to suicide but turns out she just fucked up her experiment. Kind of a relief but the fact that Qchan isn't actually her is sad. I had sort of hoped they'd be able to revive her from the ghost somehow
