For context, iirc referring to a man-female combo as yin-yang is to imply that they are having sex. Some series have the joke about "yin-yang fusion", or "yin-yang cultivation", where a horny person tries to get into another cultivator's pants.
It's very vulgar, and to suggest that a grandparent does that to a granddaughter is... yeah.
