In the manga, when Kano used it on the enemy guy, he did notice it. The manga did change a few things, such as showing the level, where the novels never did that (they only show a different warning and you can calculate based on that value, or Simple Appraisal not reading the special skills.I understand that MC's reasoning for staying low because he cared about the game's scenario. But it was still boring.
So, I'm glad that his sister is kicking ass here.
I'm under the impression that Appraisal using the magic item doesn't cause the target to notice. Is that not the case?
In Worlgermut-kun's case, the level between Narumi and him was so vast that Narumi only got the worst message, meaning that the enemy was 10 levels or higher above him, so he could not pinpoint the level, he would later make assumptions and reach an answer, but the manga outright gave him a level to make it easier for the reader.
In the special skills case, Narumi found out about his glutton skill with Simple appraisal, but the manga lagged it out, having Narumi require buying an expensive item to check on it.
In short, the manga has differences, but I believe items can be sensed just the same in the novels, it's just that you don't know if it's a simple appraisal or anything stronger.
In Kano's case, she used an item, but that gave them the impression that Kano was using one of her limited Skill Slots for a newbie skill like Simple Appraisal (remember that normal people have limited ones, and Kano has extra ones from her multiple jobs). The truth is that Kano doesn't have Simple Appraisal anymore for that exact same reason.
So this gave them an assessment that Kano was a newbie adventurer. but that's also part of how to read your enemy.
