there no need to be new actual drama. but about bring up what happen..
Though, it may not touching that subject at all, like other work. Which is not good imo.
In the other work, we know that she have a failed marriged, low self-intern. So it doesnt really need more detailed back story to start on get better.

But in this manga, the whole plot of it is how she can't tell him, he wait for her to ready on her own. Never facing the problem doesnt seem like any good, both narrative wise and for the characters

We did find out. So was the male protagonist. But not by her.

"...the whole plot of it is how she can't tell him..."
No? The plot is about her recovering from that trauma and regaining her old self by way of the happy, fulfilling relationship she has with her childhood friend (childhood crush) turned lover.

Sure, there may eventually come a scene where she tells him first-hand but, like others have been saying, the author's style is to make it so that everything's uphill from the point the main drama is revealed, which happened pretty early on here. The reveal is never the point of the plot, the point is always the healing and the fluffy relationship.

Do this need a whole reply, Lol ? i could say one piece is about some dude looking for a treasure called one piece. Yet, you could say the same how that not the plot, when that not really take a lot of it screentime, is it ? but it still the concept, the "endgame" question.
