Well, damn, was hoping for a plot twist but... eh...
For what Kiku has done, not wanting her to be alive, but on the other hand, living centuries without experienced true love twisted her mentality, just till the end and finally got what she yearned for.
It's not explicitly sown if it was her finally falling in love or the sunrays killing her or did Mahiru bit her too, making it true that vampire falling in love is it's certain end.
I truly don't like Mahiru, he didn't understood till the end his mother was not in mental state to be fully aware of her actions, this takes years to recover and he didn't even try to help her at all, doing the same as her, completely pushing her away despite he knew she loved him. Japan really does dirty on mental health.
As for their relationship, it seems like Kiku plant a memory in Mahiru when they met for the first time, but neither remember it till their death a part Mahiru.
Kiku did had her "charm" ability running 24/7, be it unwillingly or when doing the brainwashing with Mahiru mom, but even if he was left out of her reach, he kinda felt in love with her, be it through constant exposure to her ability or teenager love.
It would be fitting if Mahiru could be more older, more mature with his feelings, but in the story it kinda felt, disturbing. After all it was +400yo Kiku falling in love with 14yo with abandonence issues, that's easily to taken advantage of.
