Seeing as how there's confirmation that Snake did kill Fukuhara's friend I wondered about the reasons why.. and my guesses are that:

1. Her friend was a fellow assassin and she was killed on the job (maybe both were actively hunting Nighthawk?)
2. She was someone Fukuhara was ordered to protect and failed to do so
3. Snake decided to kill her friend after Fukuhara got in his way during a prior encounter out of petty revenge
