How bad does it get?

My bad. My wording was bad. Nobody dies, but it feels like it for one of the female characters at least in the VN. The original source (WN or LN idk) was dropped by the author so I don't think we could get a real conclusion in this story. I'll try to tell what happen next as I remember in the VN:

Itsuki
Some shit happens in the shelter. She gets kidnapped and almost got rapped. MC saves her and she begged to get sex with her desperately. Something in the lines of that she's still 'clean' and want to prove it to him. Looks like their relationship progress but doesn't show that much.
I think its safe to say that they stay together.

Makiura
At some point MC got bitten, Makiura just crumble, literally, by the news and become an 'empty shell' (words from the VN) because she knows that he will die. Then she got ghosted and we don't see much, if any, of her. Even after MC magically recover from the bite.
Really frustrating from my view, both of them were the best match imo.

Tokiko
She has her own route but I don't remember what it's about. From what we see when MC leaves her on past chapters she stills follows him.
How she knows where is he? God knows.

There is no conclusion. It felt like your copy-paste isekai open ending hopping for a brighter future.

Open, disappointing, frustrating ending.

Thanks for the details, was indeed expecting something much worse but even this feels a bit lackluster. For some reason zombie manga's 99% of the time miss the mark.
