Why would the doctor (in Korea?) not tell a couple if they're going to have fraternal or identical twins ? And more so if the couple is asking nicely...

Well, first off, this is based in China as this is a manhua. The reason the doctors don’t tell them is because how Chinese families prefer sons over daughters. If they knew beforehand, they might do something drastic especially when China limited Chinese families to only one child per family.
