nah, seems like you are new into this genre. Its called isekai'd in light novel, where the MC played the game and knows every route. Some Manga runs with the MCs intend and others are forced to play by it's plot and puts MC at a wall.

I just went to Novelupdates, and while I can't seem to find the comment you mentioned, some user also said that his brother confess to him though he didn't say if he accepted or not. If it's true then damn, being suddenly gay is one thing, but adding mpreg on top of that? Nahh its now just the writer barely disguised fetish at that point
