They're her familiars they are shown much earlier but during her whole kill the evil god adventurer they're never shown, but she uses them to spy on people.

I believe it was primarily caused by questionable hygiene on the food stall. Where the crowds of slum people (dirty) gathering together to eat food with no proper minimum hygienic procedures, like not washing hands before eating.

Since Star maiden didn't cook the food, it also has questionable quality. Even if she did cook, with that personality she had is still concerning.
