This could indicate that it’s a recent issue. Now, if it’s something natural that has been happening for centuries, then either there’s a lot of “illegal” polygamy, or, as you mentioned, there’s a lot of cheating, with many women becoming single mothers.

the other women don't vanish. the numbers don't change, even if only a fifth of women give birth

there's also divorce, remarriage, unmarried couples/throuples/etc., sperm donation, etc.

legalization of polygamy only means the state recognizes more than 2 people as being "married" in terms of tax benefits and such. nothing else. what the people actually do isn't limited by the law. nobody can stop you, even in our real world japan, to just live as a mff throuple and have children. what the people in the world of the manga do hasn't been shown in detail so far.

the only problems the 1:5 ration causes, that we've been shown or told about, is the fierce competition between women for men and the associated attitude shift. there has been nothing about birthrates so far.
