It's such a trope in manga that there must be a Japanese cultural background to it. Of course in historical times it would have been an issue in the West as well, in more conservative countries not even that awfully long ago. But it would have been more about a guy fearing the reaction from the girl's parents, especially if they were influential parents, than caring about what classmates would say. Normally if a guy got close to a cute girl, he would be proud and happy about it. The same in the other direction, obviously.
