The novel doesn't go much into the breakdown of Alter and Lilly but the personalities integrate over time in the novel. Alter's a creation of trauma, but trauma heals eventually.
It wasn't cut out, it was just moved to somewhere that it made more sense. You'll find it in chapter 59
