I would also say it's that, and he probably never put 2 and 2 together, and doesn't know about its lantern.

The dialogue and expressions on pages 36 and 37 seem to suggest that Gozu does remember, and Anko knows that he remembers, but he's just acting "cool" about it and avoiding Anko's romantic advances.

The way I understand the situation so far is: Gozu is aware of Anko's true form, and Anko knows that he knows, but she is hiding her true form simply because she wants to appear cute in front of him. Gozu is just allowing Anko to do whatever makes her feel most comfortable about herself.

The most curious line is when Anko(?) asks, "You thought it was a man, didn't you?", and "Why is that?".

I'm also not entirely sure about who says, "I will keep waiting for you no matter how long it takes.". It seems like it's a continuation of Gozu's lines (i.e.: "If you can't love your own appearance, then I will love you. [...] I will keep waiting for you no matter how long it takes.", for Anko to learn to love herself).

Was skimming previous chapters to remind myself of lore details, and in chapter 2, Anko tells us, "Your total stay in the afterlife will be equal to the length of your lifetime." So it seems that they might operate on a human time scale after all (assuming Anko and Gozu are also just human spirits).

But "Don't hang your head. Hold your head high."

The little girl spirit revealed on page 27 reminded me of Kaya.
It's interesting how this story sets up the afterlife with dead people, uncollected by the Grim Reaper Division who do not pass on, progressively rotting into mindless abominations hunting the Living. This sounds exactly like how things work in Kaya-chan, only there they are called Spectres instead of Stray Spirits here. Nice to see a little consistency across of the mythos of the author's various works.
