I can only presume that there might be an ambiguity in the original sentence of did she personally killed someone or she was a reason someone was killed. Being a widely known witch, she likely was one of the first targets of the Witch Hunt. And rather than just killing her, in order to get more info on other witches whereabouts, she might been pulled through an interrogation under heavy duress. First I thought Shien went through something similar (thus her psychological state she has to bear by smoking her medicine), but now I think why didn't the other captured witches went through something like that? And who knows? This might be the inciting incident that led to humanity turning on them. An accidental death, involving a prominent witch like Rea, turning into a misunderstanding would be just the thing
The timeline doesn't add up in this scenario - we saw the very start of the hunt in Mode's flashback, yet neither her nor Tama knew anything about the murder. If I recall this correctly, and if the translation was precise enough, Tama mentioned that not just her and Shien, but some other witches were saved by Mode's warning, meaning they kept in contact with others.
