Looking at it closer, yea, I think you're right about it being 巫女 (Miko-san).

No, I'm not sure, I sorta just filled the blank in but 'alone' isn't correct at all (definitely didn't translate this wrong becuase I got distracted looking at Kafka's thighs, no siree). Looking at it now, the translation should probably be something like "Is it okay to ___ with only Miko-san?", but the sentence can change depending on what the verb (kanji) after と is but I can't read it at all.
