None of us know. It's very unclear what's going on. But let's be real, I think if it was made clear, the manga would immediately become less interesting and a lot of people would stop reading. I think the ambiguity and sheer "What the hell is even happening?" is the main strength of this manga.

I'm pretty sure the whole thing was just them getting too into the roleplay. Idk if the two girls were getting carried away, or if they were just deliberately messing with the nurse. But I never got a hint of them being fuck buddies or anything like that. The whole thing was just a comedy routine.
