My theory is that the story is correcting the world. They can't have a brutal murder happen in this world that would ruin the plot. So the people are alive, but are given a new job to fulfil that requires them leaving.
I kind of hope Isalla isn't just killed off or given some horrid life to live. Give her some type of karma. But I actually find myself kind of liking her. She kind of comes across as a young naive child more then a hateful woman.
I would love a story with a more realistic reaction to finding another reincarnated. While the Heroine might be all giddy to be in the story. The MC has been here for probably around 15+ years. So I'd imagine she'd be on the verge of tears finally being able to talk to someone like her.

I don't know, maybe it's just me. But the MC is quite annoying. The constant " Oh I need to stay and make the same decisions of my past self" It's so obvious and for the MC who is meant to be acting like a wise older lady, she sure isn't.
I like the fact this manga seems to place itself more on the realisim side then the fantasy side. But I do kind of wish for the MC to have some magical OP powers to make such a wide variety of medicine. That'd be an interesting manga idea. Where someone takes the realistic part of medicine...
