I think there is no reason to assume this isn't after 18.2. I think combined with chapter 5, it is implied that she is thinking back to these events evertime she feels bad/sick and lies down. This doesn't need a special intro because it such a common part of her life.

And so Bocchi the Top comes to its conclusion. Like the entire manga, this final chapter was beautiful but ah so short.
