Likely because you dont think that usually. At least I do not. So until it becomes an issue I bet most people in that situation would eventually stop caring and adapt. Only thinking about the gender change when specific situations occur. Sure some may never be able to cope with it. But people tend to be able to adjust given enough time to normalize the change and if said change is not majorly impacting their environment.
Remember that the majority of people in that world dont know his real identity. So they just treat him as Mira, not an old man who now is in the body of a young girl. So most of the time there is no reason to think a out it.
