She doesn't feel pain in the moment but feels pain after the fact. She seems to be in a constant battle between her new latent sadistic disposition and her principle of not wanting to hurt people because it won't make them happy. That pain comes from inflicting violence on Misono, something that goes against a code she set for herself after being a victim of violence herself. Her eyes in that scene reflect the in-the-moment enjoyment, sure, but after she feels a guilty pleasure. Though it appears her sadism is slowly winning out.
