This might even work as a live action show. In live action it would actually be more easy to make a person look like a deranged dope addict than in manga. In manga you need to exaggerate it a lot: the subtle details the human brain is hardwired to notice mostly can't exist in simple drawings. In real life such a skill was created by evolution because before the very modern times, people had to be able to quickly judge if any stranger, or even acquaintance, was potentially dangerous or sick. Although many Japanese live action TV shows are extremely low budget and cheesy, so such a production would just ruin this. A random manga would hardly get more than such a run-of-the-mill adaptation.
