Manga does a really good job showing that while these girls are deeply troubled individuals who are not ready for authentic relationships, their guys are also no saints. Including the nicer ones like that guy from the first volume who wanted to "save" Yuki but couldn't even stand up for her in front of friends' comments, or this guy who compared Aya to a completely different woman

I think BF in this case wasn't breaking up with her just because she got plastic surgery. He was already distrustful of her, I think after hearing her age and lack of occupation the plastic surgery part was just the straw that broke the camel's back. At that point he basically concluded everything was a lie. Glad he was able to dump that trash woman.

The part where he says it's not a effort really emphasises how he doesn't understand what she went through. Like yeah she's a very insecure POS, but all of the shady work and possibly dangerous surgeries(as those often have risks involved) she did in an attempt not hate herself was definitely not a piece of cake.

Also, bruh her throwing the vape and ashtray had some weight. At least she knows a lot of it is her fault and not like generic anime characters who start wailing and crying and lashing out at others (you could argue she is doing that, but this more realistic, self-aware and non-dramatic).
