Or since we usually saw it from the villainess perspective, it looks like she just have super paranoia or weird thing happens

There's "Yowaki MAX Reijou nanoni, Ratsuwan Konyakusha-sama no Kake ni Notte Shimatta" that has almost the exact same premise as this one, complete with the low self-esteem "villainess" who told her fiance that she's doomed to be left behind by him.
