There was a manga or 3 with the same plot system though one was poorly written and roughly drawn with nothing but so side heroines and their erotic clothing keeping it forward

Another was the Mc stuck in the world and wanted to be near his sister and got into fights with angels trying to drag him to the other dimension

And the last one was just poorly written and I think the author had an idea but poor execution on what to do after the first 2 chapters

Isekai got a huge kick in the ass but thats because the genre has been getting above average quality works published now.

Honestly, every category of everything seems to suffer from this but why does it feel so pronounced with isekai?

because its more like heres a 1000 rehashes of the same thing. Look at the Otome Villainess Isekai genre and the good entries are basically Bakarina and VL99 as Comedy, The One Within the Villainess and Accomplishments of the Duke's Daughter for the Villainess being competent, and The Fed Up Office Lady wants to serve the Villainess and Gal Maid and Villainess for romance, and thats basically it for Actually good entries in the category that fit the category because a Villainess Search will notably find you:

Magical Revolution: A good lesbian romance with a minor case of Dragonball syndrome where the plot violently shifts forward because the author really isnt planning out events. 7th Time Loop: Riche is a true Renaissance Woman thrown into her own spy thriller. and Heroine Survival: Where the girlygirl protagonist has determined playing with dolls and being adorable is for cowards, she will reforge herself into Tiny Pink Lesbian Kratos and stab fate in the face.
