I don't think that's what they were going for. I think it's the opposite direction: she's such a klutz that she accidentally started writing her idol name on the form instead of her actual name (that they are somewhat similar isn't all that odd as people often pick pseudonyms that resemble their real names because they're easier to remember that way)

This is correct. Her original (family) name is "Urushibara" and she changed it to "Urumin" for her stage name. On the form, she started writing her stage name in the furigana section, probably due to habit.
