I get it in this scenario because he was literally too weak to stop her from kissing him (which him getting easily stripped was supposed to show). He didn't consent to the kiss as he had no desire for Kano. Just because a hot girl kisses you, it doesn't mean that you'll be into it/ignore your feelings for other women.

Brother had a fever, he was dazed, and some girl started kissing him outta nowhere with the pretext of "taking care of him" which made him lower his guard
Even if he wants to give up on Suzuno, it's not just some switch you can flip to move on from someone, couple this up with his fever, and the feelings of guilt spilled out, delusion or not

I mean people have a good reason to be upset about this chapter. Kano almost raped the MC and only stopped because the MC felt guilty for what was happening as he felt like he was betraying his crush. If he didn't feel bad, Kano would have likely continued and would have had sex with the MC, despite him having a severe fever and not being in a clear state of mind to even consent in the first place.
