He means the main guy. It was implied that his older sister molested him in the past. In chapter 36 there's a flashback with him as a kid with his pants pulled down and his sister standing above him naked.

Yes that's a thing that happened, but the OOP said, "Is everybody". FMC is counted as "everybody" and so far, it seems like not everyone has body dysmorphia that's triggered by sexual abuse.

I really like how she describes the scene of the protag helping the main girl stretch her legs as strange-looking for everyone else but for her it seems normal because it looks so similar to her past sexual abuse.
