Nevermind what just happened, I'm treating this like a flashback. Rather than blame anyone for the old man's escape I'd just chalk it up to plot armor on his part, he likely still has a role to play in the story, so he's not really going to die here. That makes it so that the purpose of that exchange was merely to let him know someone he wronged is out for revenge on him and he now has to fear for his life.

So this just about confirms that it's Tsukuru who's intervening to save Suzuno. Not sure about Miki since I've already forgotten if there's anyone in this crew who can heal that level of injury aside from Suzuno herself.
