I mean—there is a worst-case-scenario where he is real, but she only sees him as a doll, and thinks he isn't.

But that doesn't seem very likely. More likely, either he's real and she talks to the spirits of objects, or he exists only in her head like any normal imaginary friend, and everything's fine.
