I believe it's just a manga (or light novel) thing, and nobody will be able to convince me otherwise. Naturally I reckon people there will start by calling each other by their last names, that seems to be the culture, but I won't believe the first name thing is anywhere near as big as manga makes it look like. Of course if people never get any closer to each other, they would never switch to first names.
actually it is a real life thing as calling by first names denotes how close people are to eachother so just calling someone you've just met by their first name can feel like acting overly close

If you are seriously dating someone and still feel like you aren't close to them at all, it might be best to break up.

it is and it isn't. It is a very real thing, but like with the handholding it's become a trope in manga to MASSIVELY overstate its importance. They likely already would have been on a first name basis given how long they were friends and how often they hung out. Especially among the younger generation.

That said, senpai often is still said partially due to habit and also because it's an EXTREMELY common way to get your first boyfriend/girlfriend so the term has an almost fetishistic quality in Japanese romance. Many Japanese people have, at one time or another, dated a senpai.
