Because the author is making him out to be still a regular teenager overthinking on the possible consequences instead of realizing the moment he isekai’d any chance of a ‘normal’ life is gone.

Bro has been reading too much manga. In life and death situations like the world hes in, using any item is pretty much a gamble, especially when it's so vague. youre not gunna potentially risk your life over something that "seems like something from a book" unless you know youre in a pre written story.

I'm guessing the reason his body is failing, as he puts it, is that...

Everyone is being super firendly to him and tried to keep him included.

I mean, least we forget, he has the skills Loner and Hikikomori. Meaning he is supposed to stay alone as much as possible (he can't even party up without making them subservient instead of friendly) and be stuck in a fortress.
Basically, he is supposed to be a dungeon master. As in the old Dungeon Keeper games. That "fake dungeon", but creating one for real.

Yet everyone keeps trying to keep him among them as an equal, and keeps trying to make him hang out around with them.

We did not see any negative effect of the skills yet, besides these informed ones of his... so it would make sense that this is it.
