I feel like this is how some of the weirder youkai/ayakashi legends were made up. Some weird person was being just a little weird, and it became a ghost story over time as it spread. I mean, just imagine how "girl who faints when she's happy" could get mangled over time in a great big game of telephone. "If the Hitori Bocchi becomes happy, it makes everyone around it faint"

Is this referencing a specific youkai? One that makes people faint when it becomes happy?

People or animals with weird conditions, physical or mental, are a source of a lot of myths. Or even, for example, jackalopes are just rabbits with tumors.
