Ichika is indeed the one to say “I love you.” Shiine is saddened and doesn’t like this, which we learn from the thought bubbles we see at the end. I took that to mean that Shiine doesn’t believe Ichika would say those words to her if she didn’t have the fever. And we all know Shiine wants to hear them from Ichika.

Shiine is “upset” that Ichika told her while under the delirium from the fever, not while she was “normal.”
