I could understand flinching (kind of like when you imagine something very painful happening and snap out of it), but busting out crying seems a bit out of it; they are in a good relationship and he knows it and this was just a momentary image

but, I have to say, people unfortunately literally kill themselves sometimes for things that are, more or less, just trivial things inside their mind, so I wouldn't discount the effect imagination can have on you (especially long-term).

As we have seen many times before, Kenta has been shown to be, for a long time, somewhat vulnerable to his negative imagination, so it kinda makes sense - even if I agree that the reaction here is a bit knee-jerk all things considered

The way I see it, everyone has their moments of intrusive thoughts, maybe-not-rational fears, etc. Kenta is no different from many people in that way.

Importantly, he told Chihiro what was on his mind and brought her into his mental state. He proactively communicated instead of sitting and stewing internally. That gave Chihiro a chance to understand where he was at emotionally, and made the space for her to support him when he needed it.
