The vast majority of the time it happens, it's when the main girl looks like Shirogane (i.e., long hair, busty), and there's (often forced, it seems these days) competition in the form of a washboard/loli. MCs don't do this when the female lead looks like that other girl.

That being said: from reality to the pages of manga, it seems that it's always flat women shamelessly trying to show off how little they have.

What breasts? The fact that she doesn't have any is almost the entire reason she's in that manga.
