Reading this again after watching the movie for the first time… Hideo and Tekko’s relationship is so much more interesting and real in the manga version. She’s sweet to him and then she gets wasted and acts like a total b, lol. Hideo wasn’t schizo in the movie either, he was more like a goofy hero. I like both versions, but the manga just has so much depth.

I don't think it's definitively confirmed in the story--or at least not stated explicitly--but seeing that he has paranoid delusions, as well as audio and visual hallucinations, I would say he at least has psychosis if not full on schizophrenia.
