According to the novel, magicians basically "forged" her into a sword in an experiment. She was the only survivor of the experiment (apparently being turned into a sword is painful, lmao). My guess is that "forging" is basically extracting their soul and putting it into a sword to make the sword more powerful?
Once she became a sword, she was stuck as a sword forever. She begged god to give her back her life, so she was sent back in time. That's how she became human again.

Yes, actual sword, as in they used alchemy or whatever to turn her into the material and then they manually forged her, put her on an anvil and all of that while she felt every single moment of it. To keep her in line they used her family and their descendants as hostages .

The ML is just some kid/prince she picked to have the empire that did it to her get destroyed as revenge since she knew he was capable of it and all she had to do was let him have some motivation.

Yeah, she was a literal sword. Here are some early spoilers explaining the exposition/general plotline:

They sacrificed/experimented with her and turned her into a literal sword. She was passed down through the empire for 400 years. She hated that she was a part of the killing. As it turns out, they infused a part of god within her (?) so she started praying to God to free her and help her get revenge. She was born human, and wanted to be human again. Eventually, her wish was finally granted, and she went back in time 400 years. She wants to destroy the empire, so she plans to manipulate the prince, Dylan, who is really talented. MC is also incredibly OP, given she was a sword/experiment and gathered tons of power and knowledge over 400 years
