In other words, the brother probably loved the "parent" and would do anything to "protect" his sister. He wanted to kill the boy the sister was friends with (the fox mask person) however, missed and end up killing the elder brother of the friend. He might be the one possessed by the entity and not the sister because of the doctor and the "parent"
