Thats why Noah left dinner so angry a few chapters ago, real karen said the thing about not liking fish which pissed him off because it was an example of Cecil taking his life/role. He knows she isn't Ichika, and original karen "polluted the thing he loved the most"
OG Cecil (noah) knew she wasn't Ichika and refused to marry her/become king, so crazy queen swapped the two.
