She realized it the first time they met (When he saved her), a combination of how her treated her respectfully despite her occupation and her instinct from work which require her to pay close attention to clients (She recognized his hands and voice specifically). It's part of why she invited him to the brothel, it was more than just thanks for helping with that incident.

Remember when Kanata showed his hands on stream? Now go to Ch10 pg11; she recognized him (or at least thought she did) by his hands. Sure, Alfana did too, but she had an easier time since the mc got a bruise on his left hand and she coincidentally met him by chance.
