I thought the doctor was the poisoner at first glance, then the suspect lowered after introducing his role, but now I'm sure is him.
Most likely for his inferior skill and lack of combat power he couldn't take a role he wanted in the gang, or maybe his father got killed and he grow a grudge as they couldn't protect him, so now he wants to kill the family head and beloved grandaughter, he got no hope in a frontal fight so he have is poisoning them, after failing the first time he decided to go with a second faster attempt.
The idiot thug not remembering who he crossed is because he didn't cross anyone, doctor used insects to put the poison straight in the food, possibly when it was getting moved or in the room the kids were.
