How viable can that factory be? Unless they have a cheap and low cost cash cow that can offset the expense of running those facilities, Falma needs to start several wars so he can sell that much antibiotics to all sides of the conflict just to breakeven.

They really play up that aspect of things IRL, but it really doesn't cost anywhere near what they claim to develop or produce medication. Even just producing generics of a common medication is sufficient to keep one more than solvent; a production facility like this will be able to hit a major outbreak in a single location, but that's about it, unless the medicine derived is long-lasting and they have a stockpile built up beforehand.

On a related note, most medication is more than 80% effective 30 years after production; the expiry date on medication and the standard to destroy medication after that point is because they want to make sure you buy new medication every time. FDA wants it to have as close as possible to 100% efficiency, drug cartels want it because planned obsolescence in something that doesn't really go obsolete.

For domestic use the government (and maybe the church) will subsidize all medicine, For foreign countries, he would not avoid all of war but most of it would probably be resolved diplomatically, the empire is leading country after all, and he will sold all of "his" patents with specific terms and conditions, and attract investors to build pharmaceutical industry in each respective country

In 2021 the US was just 4th on the revenue list. The first three are: Germany, Switzerland and Belgium. Five to ten are: France, Italy, Ireland, Netherlands, UK, India. Germany alone produces more than double the medical drugs compared to the US.
The US isn't even in the top 15 for generating surplus from drugs. In fact they are the leading country in generating a deficit from drugs.

Falma already preparing his own death

Well, he is preparing a lasting legacy. Which makes sense. The foundation of his body of knowledge, after all, is the edifice of science from which he learned on Earth. The culture, techniques, and society of science work constantly to discover new things, many of which become useful for practical medicine and technology.

To discover new fungal and bacterial strains for medical applications requires the whole field of microbiology for isolating pure cultures of all kinds. Taxonomy or systematics are needed to classify and characterize them in a way that can be communicated to others around the world. Biochemistry is used to find active factors produced by those microbes, with molecular biology isolating the relevant genes that will help to create advanced production of the medicinal compounds in quantity. Statisticians are also needed to analyze test data to see if the medicines even work. Such a thing would take a century to develop, even with the guidance of a research genius like Falma. An entire generation, or several generations of people need to receive education, from elementary to post-secondary levels in order to have a few advance into scientific research, development, production, etc.
