@rudezki He's a psychopath, borderline maybe. Psychopaths lacks empathy, remorse, and guilt whereas a sociopath feels all those. In short, psychos are very detach from emotions. Sociopaths are crazy but they do not know their crazy, psychopaths do. Psychopaths are well aware of their own actions and the consequences, a sociopath doesn't.

@hikufalafel There's no difference between psychopath/sociopath. None of your descriptions have anything to do with either of those terms, and even if they did, psychologists don't even use either of those terms. Nowadays the term used for what lay-people call "sociopaths" is Antisocial personality disorder, and it's main defining characteristic has nothing to do with being "Crazy", it's about whether or not they have empathy.

Sansui doesn't lack emotions and he had some measure of empathy, he just has trouble understanding normal people after being apart from society for centuries.

@TUSF He doesn't lack emotions, else he wouldn't have picked Rain up, but they are dampened. It's not "trouble understanding normal people", he straight up doesn't feel like they do. He's said it himself multiple times: he lost human desire due to his training. So even he knows he feels differently from 'normal' people.

@hikufalafel you're completely wrong about your description, and even then your thesis is backwards, sociopaths live in a society, they fake the emotions needed to fit in.

so why doesn't he just take Rain to literally any other of the Ducal houses or even the Royal family? they have all treated him far better and with his skills they wouldn't hurt a single hair on Rain's head and he wouldn't have to be a slave. since day one this bitch has been insulting, demeaning and her family has tried time and time again to murder him, just god awful people through and through.

I disagree. If he was a newly transmigrated teenager, then you could easily make the argument. But he's a sage with complete control over his mind and body at this point. We saw him have perfectly normal human emotions up to this point. He lost desires in the Eastern ascetic sense, basically vices such as greed, lust, pride and etc., not the ability to feel human emotion. He's not written as a cold-blooded psychopath and neither does he appear as one here because his demeanor is explained by existing backstory. He's the only character in the manga for whom it would make perfect sense to stay calm during bloodshed. Also I'm pretty sure he isn't decapitating people because it's cool. He's using the guillotine approach and killing people in the quickest and most painless way possible.

Whether the writer has done a good job on making him appear or feel as someone with hundreds of years of training under his belt is a separate issue. Ideally we should have witnessed more of the training for it to register with the reader better. He does look and feel like your regular Japanese kid with the exception of being extremely calm.
