I've had the mods flag me in the past for not using spoilers on events or facts that happened in previous chapters and/or the literal chapter I was commenting on, so I'm putting spoilers just in case.
Yeah, I will admit that the writing of this plot isn't exactly the best or most exciting. With all the foreshadowing and the specific relationships and personalities of the main characters, it's fairly obvious what's about to happen.

Hint 1: Guren plays seriously and gets mad when people target his sister.
Hint 2: Guren, an artificially-engineered player, has always won in present-day challenges, and he is known as a legend in the underworld.
Hint 3: Guren wants to leave behind his dark past, but various people keep forcing him to play seriously.
Hint 4: Mizuha is just like Guren, artificially engineered to win.
Hint 5: Mizuha explicitly wants to crush Karen so that Guren will dominate her like he did back then.
Hint 6: Guren told Mizuha that the reason he won was because he (unlike her) was "not human" and thus not affected by human emotion or instinct.
Hint 7: Guren says that Mizuha hasn't changed from back then and is still "gazing at a nightmare", but she replies that she "is living a happy dream". (A blatant flag that something HAS changed about her)
Hint 8: Guren himself tried to stop Karen from accepting the duel.
Hint 9: Guren's eyes can't read Mizuha.
Hint 10: "If Karen gets flustered, the game will be set."

With all of this set-up, it'd be pretty difficult for the author to add some sort of twist or change the fate of the match without contradicting himself/herself.
The fate of this match is already set; what really matters is the AFTERMATH.
