Was this true realm stuff ever explained? Also if Chihiro's old man could personalize the swords to such a degree, why not create loads of healing swords like Suzaku for after the war? These kinds of stories always ignore that aspect, with all the magic/ninjutsu/haki going around, these societies would make massive strides in loads of areas like medicine.

It's been mentioned a couple times by now. Basically, the blades can do more than just their intended basic three abilities, but it's up to the wielders experience and creativity to tap into that potential. Seeing as Kunishige is the only one that could even work Datenseki without it blowing up on him, there is a strong implication no one really understands it - it's a force beyond their current understanding they've barely managed to harness.

That might be part of the reason why Kunishige declined to make "loads of healing swords", because some assholes probably would have found ways to make everyones eyeballs explode or some other fucked up shit. Other reasons might be that mass production is not logistically feasible, or that the process of making them actually isn't convenient enough to be able to tailor them to do anything specific, and the current blades are largely results of trial and error. There are plausible ways to handwave this.

Well yeah clearly, but that's what I have a problem with. Every shonen decides to "handwave" it instead of tapping into that juicy potential for worldbuilding. It's like they're scared of anything that can't be framed in terms of battling. The titles that do embrace this are that much better for it. HxH and NGE come to mind.
