I will probably regret being semi-serious for even a moment about a manga like this, but I'd like to point out to everyone that she has no communicated the "love" for her brother to him in any way. You can't really blame someone for not reciprocating feelings they don't even know exist, especially since they're siblings so it's natural to assume they don't.

No she has. She's been very physical touching him, hugging him, kissing him on the cheek. It's just that he takes these actions from her as her being a little sister showing close-contact sisterly love to her brother.

He doesn't understand that her love and the love he thinks she has are very different things and there's no realistic way for him to get it unless she literally goes mask off and be explicit in exactly what she wants and feels.
