Ok hear me out. The person is Yotsuha from a different timeline. With the clover, the fact she mentioned she was one of the ones who killed him the first time around, and the fact the person who wants revenge for someone being killed. So maybe it's weird time bs where she's getting revenge on her self for being the one to start this whole mess.

I has a similar idea tbh but I didn't come up with a reason.
Either way I think uichi isn't the persons main target of revenge even if they are the person targetted I think whomever it is sees Yotsuha as the end goal whether that's love or hate I'm not sure of

It can't be Yotsuha because she goes back in time via regression, there aren't 2 of her. The only clue we really have is that it said they want revenge on that bastard by any means necessary, this doesn't specify Uichi as the target so I do agree it's likely someone after Yotsuha using Uichi as a means to an end as Yotsuha is the only person that truly cares about him anymore.

What if Yotsuha travels back to a time and it displaces her other self or it's a reality where her other self has died or merged with the time travelling version? Or a switcheroo type where she takes the place of another Yotsuha and the other is left to fend for herself in a new reality?

Ok... I'm dropping a new theory that includes spoilers for following chapters, do not fucking read unless you want to get spoiled hard
we'll find out soon what happened to yotsuha when she moved away, how much of a shitty life she had and how her mom was a piece of shit that leaves uichi's dad like a saint, but also we'll learn that she got really sick, a heart condition iirc, I had been wondering what could uichi had done to warrant a vengeance through space and time that made someone try to destroy his soul and make him kill himself, but... What if it's yotsuha the one who dies later in the story from straining herself too much saving him? Then clover (whoever he or she is) just blamed uichi and killed him in the past creating an infinite time loop? Uichi dies -> yotsuha saves him-> uichi survives -> yotsuha dies -> clover swears revenge -> uichi dies... and we're back at the beginning
