Definitely her brother ngl. Probably because he wants to protect Yotsuha, and right now she is sacrificing herself to save him as far as I can tell due to the heart surgery. He doesn’t question the time travel, and I think he is trying to protect Yotsuha by removing Uichi, as fate dictates only one of them can live or something. Have a feeling we will have a very sad ending, where only one of them are alive, or they both die. (Not too sure about my last statement lol but very sure about everything else).

In the last chapter I predicted that Clover was probably her half brother, but not the brother we were introduced to previously. I think this strengthens my theory quite a bit! Clover is most likely her half brother from her father's side, and doesn't want to harm her directly because they're related. Though, I'm not sure who he could be referring to when saying that they look alike - is there more to the hidden family we don't know about?

For people who misunderstood the "guess they really are related" line. That mail was for her brother probs. I think the main villain is either her brother or maybe some other family member she doesn't know about. It definetly feels like the person is older than the kids though, if he spoke the way he spoke to Uichi's sister, I am guessing it isn't someone young. Man, this manga is actually so good I can't. Genuinely, made me real emotional, a lot. And I hope that everything goes well (The tragedy tag concerns me, but I hope it's only the tragedy where it was shown tragedy and won't influence the ending.) Sorry for the yap and thanks for the translation

He is (or more likely related to) someone her mom ruined. That's my bet. Won't attack her(Yotsuha) directly because she needs to be alive to suffer.
Also laboring under the delusion that the psychopath mother would give any degree of a fuck if Yotsuha suffers.

Unless that's supposed to be someone we recognize. 🤷‍♂️
That the guy that got bullied in middle school to told uichi to kill himself in the original timeline
