If that was the case, I feel like coming clean to her might work at this point, or even before this, so I doubt he's related to the clover person. My bet is he's just worried for her, and wants her to give up before she dies.

Skimmed the raws a few weeks back, but after reading the translated version of this chapter, I now mostly believe it wasn't the brother.

I'll be honest, I could be delusional, but this kind of feels like diversion, too.
If this was supposed to be the reveal I think we might have seen him destroy the watch in a dramatic moment or something (and even then we might not be fully sure).
We haven't actually seen him do anything other than stare yet, so it's more likely a red herring cliffhanger than the brother being Clover.

hey calm tf down clover is probably trying to protect Yotsuha, as there’s probably some shit like only once person can survive in this timeline (Uichi or her), or at least they think they’re protecting her. Clover can time travel as well (I think) so they’d know this, and Yotsuha doesn’t bother with a future past Uichi.
Doesn’t mean clover isn’t evil tho maybe it’s another random dude from Uichi‘s past who‘s manipulating big bro.

There is not even a single indicator so it wouldn't even count as red herring. He is literally just waiting for them to have their moment outside lol. He is though, at the very least, aware of the danger yotsuha is in since he is the legal guardian and knows why she is delaying the surgery. Probably not very happy about it
